What to record when a decision needs an audit trail
A plain-language guide to recording operational decisions so the business can understand what happened without creating unnecessary administration.
Record the decision that changed the work
An audit trail is not a diary of every conversation. Start with the decisions that approve, reject, alter or release work in a way that someone may need to understand later.
This keeps the record focused on operational accountability rather than collecting details no one will use.
- Name the person responsible
Keep the essential context together
A future reader should be able to see what was decided and why without searching across several systems. Capture the decision alongside the related request, job or customer reference.
- The decision and its effective date.
- Who made or confirmed it.
- The reason, evidence or policy that supported it.
Make the record proportionate
Not every task needs the same level of detail. Match the record to the consequence of the decision, especially where cost, commitments, compliance or customer impact are involved.
